We have found a lot of variation chain noise in these engines. Often when people have chain repairs done the shops take a shortcut and change only the tensioners. It is also common for shops to change the two cam chains and leave the shorter chain to the high pressure pumps, because that requires engine removal. We have seen a number of these trucks that had budget cam repair, where the whole job had to be done over because the shortcuts left a noisy motor. I'm not saying that is your problem, because I have not heard your motor, but it is a possibility.
As others have suggested there is also injector noise and other mechanical noises from the belt driven accessories

I had thought you could do the accessory drive chain at the same time as the cam chains.
