small antifreeze leak near back of engine

Radiator hose "plug"
The large hose that runs over the top of the fan and it appears into the radiator is bubbling fluid out if it where it appears some sort of plug was. I found the plug sitting on the fan cover next to where it goes into the hose. Any info on what could have caused this and what it takes to fix it... -
