That type of failure to the cam sprockets are not uncommon.
A rattle on cold start ups that goes away after a few seconds is an indication of eminent failure.
I really don't see metal flakes in the oil.
Maybe change the oil a couple of times every 500 miles.

When I do the trans pan.
I remove the transfer case crossmember, the right side motor mount nut, the air intake tube, and the upper fan shroud cover.
Jack the motor up from the right side, this will give the room needed to remove the pan.
That connector is for the battery saver module. Used during shipping.
The battery saver module is removed when the pre delivery inspection is performed.
There are two of these elbows, Both in the front foot well area, Driver's side is up about 12" above the dead pedal. Right side is right next to the fuse box. Pull the right kick panel cover for better access.
Could be the windshield cowl too. This can be checked by lowering the glove box and inspecting around the cabin filter.
Usually its the front sun roof drains inside the front footwells. There's a rubber elbow the deteriorates and leaks.
The replacement elbow can be bought from VOLVO for a few...
That's a lot of money to remove the intake manifold. Comes off in less that 10 minutes.
But, the manifold can be lifted up at the front to access the Y pipe.
