Almost sounds as if the brake pedal switch is bad or disconnected. Its weird though b/c the cdl light is on. I thought that the light won't come on without cdl shifter in a DII.
Congrats on your purchase. The DII's have a smaller lug pattern, something like 5 on 4.5 or so, so you can't use DII's wheels.
Just curious, what was your build date? Mine's March of '98 and is a '98 model.
The cdl freezing or becoming stuck seems to be a common problem. Try searching for CDL. You should find your answers there. Try this site for reference and good luck.

Here's my stock '98, which I bought in June. The pic was taken at the Prairie City SRV park near Sac. So far, I haven't done much to the truck except cutting the front bumper, removing the stock grill guard and side steps plus changing all of the fluids and sparkplugs...
I know you've been under there already but I would check it again. Check all of your suspension, steering and exhaust components (one of my muffler hangers is broken and makes a little knocking noise). Good luck!
I have the same thing goin' on with mine too. I just changed the fluid to Mobil 1 and added a bottle of Lucas. The noise is still there but slightly less loud. Sorry I couldn't help much. Anybody else have any ideas?

In your '99, there will be a nipple on the t-case for you to add the cdl to your truck. DiscoMike will be able to tell you where to find it on the t-case.
I didn't check all of the exhaust manifold bolts but the few that I did check were solid. I could not find any cracks either. There is a leak on the right side, where the four pipes connect into one. I plan on having that fixed soon, so that will help with the noise issue.

I'll pull the pan, hopefully, next oil change. Should I do my next oil change sooner to see? I haven't had any problems with the oil light and I check the level often.
Further, should I use a thicker oil on the next change to help quiet the clatter? Or, does that even matter? Typically gets into the 40's overnight here in the winter with an occasional drop into the 30's.
